More Than a Form
Because we are a chiropractic practice, a sports physical here also means someone is watching how your athlete moves. If something in their alignment or range of motion is likely to become a problem over the season, that is worth knowing in August rather than October.

What to Expect From the Visit
The visit is a straightforward screening appointment. Bring the form your school or league requires and any relevant history, and we work through it with you. Most appointments are short, and families commonly come in together when more than one athlete needs clearing.
You leave with the completed form. If something during the screening needs a closer look, we tell you and point you to the right provider rather than signing around it. Where an athlete is already dealing with an old injury or a movement problem, we can talk through whether ongoing care would help, but that is a separate conversation from the physical.
What the Sports Physical Covers
The sports physical covers what the participation form requires so an athlete can be cleared to play. It does not cover general health screening, ongoing medical management or anything outside that form, and it is not a substitute for a primary care visit. Keep your regular physician for that.

How long does a sports physical take?
- Most take a short appointment slot rather than a long one. Bringing the completed paperwork and any history with you is what keeps it quick. Families with several athletes are usually able to book back to back visits.
Do you use the school or UIL form?
- Bring whichever form your school or league requires and we complete that one. Different districts and leagues use different paperwork, so the form you were given is the form we work from. Call us first if you are not sure which you need.
Can you fit us in before the deadline?
- Usually, yes, though the weeks before season deadlines are the busiest. Call 817.447.1414 with your deadline and we will find you a slot. Booking early is the safest option once the sign-up window opens.
